REQUEST FOR A CERTIFICATE OF INSURANCE

There are two ways to obtain your certificate.

1. Download fill in the document and email it to your USDA State Representative
2.  Go to your club on the Portal and scroll to the pag that gives you the opportunity To request the certificate.  It will be emailed to you.
